Perfusion Accidents and Lessons from the Cockpit
Session Objective:

    To review the current incidence of device failures and human errors related to cardiopulmonary bypass. Making the most of mishaps or near-misses will be discussed. How to change the culture in the operating room to improve safety will be presented along with lessons from the aviation industry.
