Reminder to Apply for eRx Program Hardship Exemption
Beltway Briefings, October 2011 -- Physicians who are eligible for the eRx Program, but who prescribed fewer than 10 times between Jan. 1 and June 30, 2011, must apply for a hardship exemption before Nov. 1 in order to avoid incurring penalties. When applying for a hardship exemption, an eligible provider will be asked to submit the following information to CMS:
• Identifying information such as the TIN, NPI, name, mailing address, and e-mail address of every affected eligible professional;
• The significant hardship exemption category(ies) that apply to the application;
• A justification statement describing how compliance with the electronic prescriber requirement during the reporting period would result in a significant hardship to the eligible professional or group practice; and
• Attestation of the information provided.
