  If you are adding a surgeon to your site, please submit the contact form above along with their signature on a Schedule A:
  
  Schedule A - Surgeon Signature Sheet - Required along with the Contact Form for each surgeon being added.  Please indicate "General Surgeon" for each general surgeon being added to the General Thoracic Surgery Database.
  
  Remember to report any software vendor changes to STS.   Email Gerry Tarafa at gtarafa@sts.org if your site has changed software vendors.
