>>>>The STS Data Collection Form (DCF) for the Congenital Heart Surgery Database - Version 2.50 has been recently updated.  The version number on the DCF remains the same, version 2.50, but the text "(Updated 7.10/2006)" is included to show this is the most current version of the document.  All parties using the DCF should make sure that they delete any copy of the DCF that does not include "Updated 7/10/2006" at the top of the document.<<<
