Temporary Fields Implemented - May 6, 2020

To help further understand the impact of COVID-19 on surgical patients, STS will collect the date of positive PCR testing for COVID-19 patients with surgery dates starting May 1, 2020. Sites have the option to code this field retroactively back to January 1, if they choose to do so. 

To achieve this, the temporary field (TempDt) will be utilized for patients who have a confirmed COVID-19 diagnosis through PCR testing.

The training manuals and data collection forms have been updated accordingly and posted on the STS Database webpage for each database component. Please note that temporary fields are intended for STS use only. Do not enter data into temporary fields other than as instructed by STS.

Intermacs/Pedimacs COVID-19 - April 1, 2020

The STS Intermacs Database and its Pedimacs component have follow-up visits at 1 week, 1 month, 3 months, 6 months, and every 6 months thereafter. Beginning at 6 months, the visits have a 2-month gap before or after the follow-up visit. Due to the current situation involving COVID-19, please use the following guidance for those follow-up visits:

  • Encourage televisits - if a visit is conducted outside the follow-up window, enter the last day of the generated follow-up visit date
  • Conduct quality of life (QOL) surveys via phone or mail

We are in the process of adding data fields to capture COVID-19 on the pre-implant form, follow-up forms, infection form, and death form. Additionally, Q4 reports will be distributed on April 10, 2020. The reporting team at the University of Alabama at Birmingham successfully incorporated to these reports several changes requested by sites. Some of the additions include: 

  • Neurological event category breakdown
  • Neurological event breakouts
  • Patient percent on all rates
  • Table for competing outcomes with estimates at multiple time points

Data Collection - April 1, 2020

Due to a reported increase in poor surgical outcomes for patients who are COVID-19 positive, the Society recommends that all sites collecting data for the STS National Database begin gathering COVID-19 information for all patients who underwent a surgical procedure on or after January 1, 2020. Sites can collect COVID-19 cases back to January 1, as needed. STS does not expect sites to code this field retroactively for patients not impacted. If this field is left blank it will assumed to be No.

To achieve this, the temporary field (TempCode) will be utilized. This field will have five options using Harvest Codes 10-14*. To code Yes, the patient must have had a laboratory confirmed diagnosis, not a suspected diagnosis.

  • Did the patient have a laboratory confirmed diagnosis of COVID-19?
    • No (Harvest Code 10)
    • Yes, prior to hospitalization for this surgery (Harvest Code 11)
    • Yes, in hospital prior to surgery (Harvest Code 12)
    • Yes, in hospital after surgery (Harvest Code 13)
    • Yes, after discharge within 30 days of surgery (Harvest Code 14)

GTSD version 2.41, use TempCode Sequence 4080

  • Did the patient have a laboratory confirmed diagnosis of COVID-19?
    • No (Harvest Code 10)
    • Yes, prior to hospitalization for this surgery (Harvest Code 11)
    • Yes, in hospital prior to surgery (Harvest Code 12)
    • Yes, in hospital after surgery (Harvest Code 13)
    • Yes, after discharge within 30 days of surgery (Harvest Code 14)

CHSD version 3.41, use TempCode Sequence 6724

  • Did the patient have a laboratory confirmed diagnosis of COVID-19?
    • No (Harvest Code 10)
    • Yes, prior to hospitalization for this surgery (Harvest Code 11)
    • Yes, in hospital prior to surgery (Harvest Code 12)
    • Yes, in hospital after surgery (Harvest Code 13)
    • Yes, after discharge within 30 days of surgery (Harvest Code 14)

The training manuals and data collection forms have been updated accordingly and posted on the STS Database webpage for each database component. Please abstract using the corresponding Harvest Codes until the vendors are able to implement these values into their software.

*Harvest Codes 10-14 are being used because of the unexpected data sites entered into these fields. Please note that temporary fields are intended for STS use only. Please do not enter data into temporary fields other than as instructed by STS.

Delayed Harvests, Public Reporting - March 27, 2020

STS remains vigilant in monitoring the coronavirus and its impact on organizational activities, upcoming events, and — most important — our members and database participants. We understand that many of our surgeon members are grappling with reduced caseloads and others are taking on new clinical roles, while data managers are being redeployed to provide clinical care on the frontlines. 

Although we continue our efforts on the phased rollout of the next generation STS National Database, our top priorities are patient care and ensuring the health and safety of our loved ones during this unprecedented crisis. As such, we are writing to announce the following changes regarding STS National Database initiatives for 2020 and related timelines.

Data Harvests

  • The first 2020 harvest for the Adult Cardiac Surgery Database (ACSD) in the IQVIA platform was completed. Harvests 2 and 3 will be combined, so the next harvest deadline will be August 2020.
  • The first 2020 harvest for the General Thoracic Surgery Database (GTSD) also was completed. The next harvest deadline is mid-September 2020. No adjustments will be made.
  • Congenital Heart Surgery Database (CHSD) beta testing is under way. While the CHSD official launch date remains to be determined, an update will be provided to CHSD participants in the coming weeks. Please be assured that ample harvest time will be provided. 

STS Public Reporting Online

  • ACSD and GTSD data refreshes will be postponed until January 2021.
  • 2020 updates to the CHSD public reporting pages will proceed as announced last November by STS leadership. The exact timeline is to be determined.
